Teen’s Life Saved Through Multi-Disciplinary Efforts at AIIMS Bathinda

In a remarkable display of medical expertise and teamwork, a 17-year-old male victim of a severe road traffic accident on 20/06/2024 was successfully treated at AIIMS Bathinda. The patient, who arrived at the triage at 6:30 am with Grade 4 haemorrhagic shock, underwent life-saving interventions following immediate resuscitation efforts.
The initial treatment included the administration of fluids and blood transfusion. However, the patient remained hemodynamically unstable and was taken for damage control surgery. This procedure included exploratory laparotomy, peritoneal lavage, the Pringle manoeuvre, and packing with 25 mops over the perihepatic region, along with ICD insertion. The surgical team successfully controlled the bleeding, draining approximately 4.5 litres of blood intraoperatively. The patient was managed with multiple blood transfusions during the surgery.The patient was managed with multiple blood transfusion and monitored continuously during the surgery by anaesthesia team.
Following the surgery, the patient was transferred to the ICU for intensive monitoring and continued resuscitation. Over the next few days, he required further blood product transfusions and was re-explored on June 22, 2024, for the removal of packed mops from the perihepatic region by the trauma surgery team and the insertion of a modular external fixator for a right-side femur fracture by the orthopaedic team.
The patient showed significant improvement post-surgery and was extubated on June 27, 2024. He was transferred to a normal ward and discharged after four weeks, having made satisfactory progress.
The successful management of this complex trauma case underscores the critical role of prompt triage, multidisciplinary teamwork, and advanced surgical interventions at AIIMS Bathinda. The collaborative efforts of the trauma surgery, anesthesia, orthopaedics, and SICU teams ensured comprehensive care and a positive outcome for the patient.
