The Department of Bariatric Surgery at Fortis Hospital Mohali has given a new lease of life to several patients who are morbidly obese(weighing more than 100 kg) and suffering from obesity-related diabetes, hypertension, and other co-morbidities through Metabolic Surgery, also termed Bariatric or weight-loss surgery.
A 50-year-old woman, weighing 105 kgs had been suffering from diabetes, hypertension, high cholesterol, osteoarthritis and sleep apnea. She had also been experiencing frequent urination, lethargy and general malaise. As her health condition worsened, she finally approached Dr Amit Garg, Consultant, Bariatric Surgery, Fortis Hospital Mohali, Dr Garg examined the patient and following detailed medical examinations, suggested Metabolic Surgery as the appropriate line of treatment.
Metabolic Surgery, also called Bariatric or weight-loss surgery, involves surgical alteration of the stomach, intestine, or both to aid weight loss in patients with obesity-related metabolic disorders.
During the press conference, while discussing the cases, Dr Garg, said, “According to the recent guidelines by several Diabetes Associations (American Diabetes Association, International Diabetes Federation, Diabetes UK), Metabolic Surgery is the first line of treatment for obesity in patients, both with and without Type 2 diabetes. It benefits patients with BMI of > 37.5Kg/m2.The aim is to put it in remission, defined as normal blood sugar level.Patients need not go in for diabetes medication.”
On the benefits of Metabolic Surgery, Dr Garg, said, “It helps cure Type 2 diabetes by causing calorie restriction, hormonal changes such as reduced Ghrelin, increased GLP1 and peptide YY. It helps reduce a patient’s risk for diabetes, hypertension, heart disease, arthritis and cancer. Metabolic surgery is also useful in non-obese Type 2 diabetes patients. Loop Duodenojejunal bypass with Sleeve Gastrectomy and proximal jejunal bypass with Sleeve Gastrectomy are novel metabolic surgical procedures to treat diabetes.”

Dr Garg further said all the surgeries are performed laparoscopically, which means they involve making several small incisions that are closed with only a few stitches, resulting in fewer complications and quicker recovery for patients.
On the traditional first line of treatment for Type 2 diabetes, Dr Garg, said, “Weight loss, healthy diet, regular exercise, and medications help the pancreas produce more insulin or help the body use the insulin that was produced efficiently. If these drugs don’t work, a person needs to take insulin. Even with good control of diabetes, some patients still develop cardiovascular complications, diabetic foot, neuropathy and nephropathy, retinopathy and other complications.”
